Wrong ideas and mental illness/problems

What's the difference? How does one distinguish between the two?

I suspect the difference is the strength with which the ideas are entrenched, to steal your choice of word. A wrong idea (assuming we're smart enough to make sweeping judgements about what's right and wrong) that is easily changed is not a problem; one that is actually just the rational disguise of a strong underlying conditioned reflex is quite different..
